Active Podiatry Terms and Conditions
At each initial consultation the Podiatrist will need to take full details including a medical history. This information enables us to give you the best possible and safest level of service. For this reason it will not be possible to continue with your treatment if this information is not provided when requested.
All treatment options are fully discussed and outlined during your consultation including estimated costs. These costs are estimates only and may change should the treatment plan need to be altered.
If you feel that you need a chaperone we are happy to provide this on request but may need to reschedule your appointment in order to arrange this.
In some circumstances it may be possible to see children without the presence of a parent or guardian, but this must be agreed with the parent/guardian and is at the discretion of the Podiatrist.
Length of appointment times are for guidance and diary planning purposes only. Depending on the treatment requirements actual appointment lengths may be shorter than the allocated time slot. It may be necessary to schedule further appointments to finish the treatment plan if there is insufficient time at the scheduled appointment. Refunds will not be given in the event of the actual appointment length being shorter than the allocated time slot.
Missed appointments or appointments cancelled without 24 hours’ notice are charged at the full rate for that appointment. Giving reasonable notice of cancellation helps us to offer that appointment to someone else.
All information shared with the Podiatrist in connection with your treatment either verbally, written or recorded on media is confidential and will not be shared with any third party without permission.
However, we may feel it necessary in support of your treatment to contact your G.P or other health professional involved in your care and share details of your treatment. If we need to do this we will ask your permission beforehand. You have the right to request that we do not share this information; however, depending on the circumstances, we may find it necessary to discontinue your treatment if we feel that withholding this information may adversely affect your care.
All appointment fees must be paid in full at the time of the appointment by credit card, debit card or cash. This includes referrals from medical and other insurance providers with the exception of those we have a pre-existing agreement with. In those circumstances you are liable to pay any outstanding fees not covered by your insurance policy under their terms and conditions.
